Cost: $200 per player (if proof of insurance is provided; $250 if on-ice insurance is needed)
Cost includes: Ice time, socks, tape
Payment Terms: Full payment is due by August 10, 2012. Credit/debit cards or checks will be accepted.

Itinerary:
Friday, 24th: 4PM-6PM GAME
Saturday, 25th: 4PM-6PM GAME
Sunday, 26th: 11AM-1PM FINAL GAME
Players are required to bring their own equipment; skates, sticks, helmet with half shield. Socks, jerseys, and tape will be provided. On-ice insurance is MANDATORY. If you do not have proof of insurance, on-ice insurance will be provided for you for $50 extra.
